How to register

Please see the 'Online Registration' tab above for more details of how to register online.

Alternatively, you can complete both of the forms below and bring these to Reception. You will need to bring photo identification (such as a passport or driving licence) to the Surgery should you wish to use our online services. Our reception team will then complete an application for online access on your behalf and will supply you with your login credentials and details on how to access the website. You will need to log in to this account within 10 days of activation. Otherwise, the account will be locked for security purposes.

If you are applying for access on behalf of a child, you will need to bring in identification for both of you (the identification for the child can be a passport of birth certificate). If the child is aged 10 or more, then they will need to bring their ID themselves.

In the Practice area

Everyone registering with the practice will be asked to provide evidence of their:

  • identity (e.g. passport, birth certificate or photocard driving licence)
  • address (e.g. a utility bill)

Please speak to the reception team if you are not able to provide this evidence. 

We have an open list and welcome requests for registration from patients living in or moving to the practice area. You can register by bringing along your NHS medical card or completing a form available from reception. We recommend that new patients undertake a health check with a Healthcare Assistant. You may request be registered with a specific GP and we will endeavour to comply with your request wherever possible.

Out of Area Registration

Out of area registration (with or without home visits) is voluntary for GP practices meaning patients may be refused because they live out of area.

If your application is considered the GP practice will only register you without home visits if it is clinically appropriate and practical in your individual case.

To do this we may:

  • Ask you or the practice you are currently registered with questions about your health to help decide whether to register you in this way
  • Ask you questions about why it is practical for you to attend this practice (for example, how many days during the week you would normally be able to attend)

Change of Personal Details

If you change your name or address or mobile phone/phone number, please inform reception for updating as soon as possible.  If you move outside the practice area you will need to register with a more local doctor.

Named Accountable GP

All patients at Nunwell Surgery have a named, accountable GP who is responsible for patients’ overall care at the practice. Your named GP has been allocated to you by the practice.  If you wish to know who your allocated GP is, please contact the practice. You can still talk to or make appointments to see any of our doctors or   nurses. We will endeavour to comply with any reasonable preferences expressed. If you are not happy with your allocated GP you may express a preference for a different GP and we will try to accommodate request wherever possible.
